Vocational Rehabilitation

Eligibility Criteria

  • Must be a member of a state or federally recognized tribe

  • Must have a documented report of a physical or mental disability which results in a substantial impediment to employment

  • Must be able to benefit from an employment outcome that requires Vocational Rehabilitation services to prepare for, enter, engage in, or retain gainful employment.

  • Must reside on or near the Muscogee (Creek) Nation tribal boundaries

Applying for Services

 

A Client Information Packet and a Document List will be mailed or given to you to fill out.  You will be asked to bring them in when you meet with your counselor.  It will be helpful if you have the following:

  • Proof of Indian Blood

  • Proof  of Social Security Number

  • Proof of Age

  • Proof  of Address/Residential Verification

  • Proof of disability:  reports of recent medical examination, school records or other information that you feel may help the counselor understand you disability.  If you do not have these a release of information will be signed and your records will be obtained by the program
