Position:  Utilization Review/Discharge Planner - CNCH

Starting Salary:  $19.11 to $27.52 per hour

DESCRIPTION OF ASSIGNMENT: The purpose of the position is to review the utilization of hospital resources against established criteria documentation requirements of external agencies. Monitor and evaluate the medical necessity, appropriateness and efficient use of health services. Provide a communication mechanism to assure the continuity and quality of patient care, to identify patients in need of discharge planning, to implement and coordinate a feasible plan of helping the patient and family in post-hospital care. Incumbent is under the supervision of the Director of Nursing (DON). The Muscogee (Creek) Nation Health System provides comprehensive health care services to Native Americans living within the Muscogee (Creek) Nation through the Muscogee (Creek) Nation Community Hospital (CNCH) and five (5) ambulatory health centers in Eufaula, Okemah, Okmulgee, Sapulpa, and Coweta.  The Health System also maintains eight (8) Field Health operations consisting of the Behavioral Health Services (BHS); Vocational Rehabilitation Services; Contract Health Services (CHS); Community Health Representative (CHR); the Elderly Nutrition (ENP); the Women’s, Infants and Children (WIC) Programs; the Tobacco Prevention Program (TPP);  and the Diabetes Prevention Programs.

REQUIREMENTS: Candidates must meet the following requirements:

An Associate Degree in Nursing (ADN) received from an accredited nursing program or Bachelors Degree in a healthcare or social service related field is required. A Bachelor’s Degree in Nursing (BSN) received from an accredited nursing program is preferred. As applicable, current State of Oklahoma Nursing License is required.

     

EVALUATION METHODS: Applicants who meet the requirements described above will be evaluated to determine the extent to which they possess or have the potential to acquire knowledge, skills, abilities and personal characteristics, (KSAP's) required of this position. Applicants should address the following KSAP's on a separate attachment to their application.

 

1.   Knowledge of patient registration systems and of the Health System in the provision of basic health services.

2.   Knowledge of benefits offered through third-party resource programs such as Medicare, Medicaid, Department of Veterans Affairs, tribal benefits packages (Creek and non-Creek) and other resources, etc. available to eligible patients.

3.   Ability to develop, implement and evaluate a patient care discharge plan to meet the need of assigned patients.

4.   Professional knowledge of basic health care principles, ethics, practices and processes required to assess the needs of assigned patients.

5.   Practical knowledge of services and facilities offered by other community resources and procedures necessary for referral and follow-up.

6.   Knowledge of the Privacy Act in regard to confidentiality and the release of medical information.

7.   Skill and ability to organize and multi-task – ability to prioritize tasks by importance.

8.   Ability to communicate courteously and effectively with patients and their family members, co-workers, medical care providers, management personnel, tribal leaders and the general public both orally and in writing.

9.   Skill in the use of a personal computer in a Windows environment, and other office equipment.
