PROGRAM OBJECTIVE

The Emergency Medical Assistance (EMA) Program is a Tribal Program that provides assistance to Creek Citizens in obtaining prescribed medical equipment necessary to sustain health and well being when no other resource is available. This program is designed to assist the Creek Citizens and isn't intended to cover the total cost of prescriptions and related expenses.

 


ELIGIBILITY REQUIREMENTS

v     Applicants must provide proof of Citizenship by furnishing a copy of their Tribal Enrollment Card. Any minor under the age of one (1) shall have an enrolled parent and be enrollment eligible.

v     Applicant must be on file as a patient OR establish a patient file with Muscogee (Creek) Nation Health System.

v     Applicant must present the original prescription and a statement from the Tribal or Indian Health Service Health Facility that the prescription or a reasonable alternative prescription is unavailable at those facilities.

v     Applicant is to make an application and have prior approval through the Medical Assistance Program on prescriptions that are obtained outside the Muscogee (Creek) Nation Health System.

v     The applicant must furnish documentation, which verifies all other available resources (Medicaid, Medicare, Private Insurance, etc.) have been exhausted.

 


BENEFIT AMOUNT

v     A grant of $1,000 per year (365 days YTD) for Health Maintenance medications and Medical Equipment. (Oxygen, concentrators, nebulizers, walkers, hearing aid, etc.)

v     A grant of $2,500 per year (365 days YTD) for Life Sustaining Medications only (cancer, dialysis, heart related, etc.)

v     A grant up to $4,000 for eligible applicants (one time only) for prosthesis (artificial limbs)
