72 Hour/30 Day Emergency
Call-In Request
As of April 1, 2004, Call-In Requests were given
a lower priority and will be denied, and can be appealed.
**However, special appropriation has been funded
to assist only Creek Citizens with the emergency request, but still have to
meet the following requirements.

When a patient has an emergency and is unable to go
to the nearest Creek Nation hospital/clinic or an IHS facility, patient or
their representative MUST do the following:
- Call Contract Health Service office within
72 hours after the beginning of treatment or admission at a private
health care facility.
- Section 406 of the 72 hour notification is
extended to 30 days for elderly Indians 65 years of age or older and
disabled Indians who have a limiting physical condition that prevents
them from calling.
Patient will receive a letter requesting the following
documentation to the CHS Central Office within the established timeframes
prior to determining the medical priorities:
- Medical Discharge Summary and History and
Physical Report
- Copy of Emergency Room Record (ER visit only)
- Copy of Ambulance or Lifeflight report
- Motor Vehicle Accident Report
- Evidence of use or denial of other resources
- Eligibility must be established by submitting a
copy of Certificate of Degree of Indian Blood (CDIB) from a federally
recognized Indian Tribal Agency or official documentation from your tribe.
Denial Process/72 Hour Call-In Request
- A denial letter will be sent to notify the
patient, and allows 30 days to appeal.
- Same appeal process for referrals applies for
the 72 hour call-in request.
