Appeals Process

Appeals committee meets monthly to review all written appeals which have been filed.

Patient and/or provider have 30 days from the date the denial letter is received to appeal for reconsideration.  Appeal should be made in writing and mailed to:

If decision is upheld, the next level of appeal can be made in writing within 30 days to:

The CEO's decision on appeals shall constitute final administrative action.
