Employee Benefits Package The Health System offers a full range of employee benefits in the Employee Benefits Package. The Benefit Package includes a comprehensive health insurance plan, a generous 401(k) plan, annual leave, sick leave, paid holidays and other various various leave benefits. Medical Insurance The Health System provides employees with an extensive health insurance plan at no cost to the employee. Employees may cover all eligible dependents for a nominal fee of $73.00 per month. Coverage includes a wide range of medical services, dental services, vision services, prescription drug card and short-term disability. 401(k) Plan The Health System provides employees with an outstanding retirement program. Employees are eligible for the plan after one year of continued employment. The Health System contributes 5% of gross salary for all eligible employees and matches employee contributions up to 4%; contributing a maximum of 9% of gross salary. Additionally, employees may contribute up to 20% of gross salary into the plan before taxes and may customize their investment elections among various investment funds. Life Insurance The Health System provides life insurance to eligible employees free of charge. Employees are insured for an amount of two times their annual salary, with a maximum benefit of $100,000 life insurance. Dependent life insurance is available for a spouse and/or children. Annual Leave The Health System provides employees with annual leave based on the following schedule: 0-3 years > 13 days per year 3-15 years > 19.5 days per year 15 years & up > 26 days per year
Sick Leave The Health System provides employees with 13 days per year of sick leave. Employees may accrue up to six months of sick leave. Holiday Leave The Health System currently has 13 paid holidays per year which include: Native American Day Veteran’s Day Thanksgiving (2) Christmas (2) New Year’s Day (2) Good Friday Memorial Day Creek Festival Holiday Independence Day Labor Day
